Our Approach
We aim to provide tailored solutions for weight management based on your medical condition, cultural preferences, and baseline body weight. We have created our programs based on small, bite sized behavior changes that are guided by health professionals and are easy to fit into your current lifestyle.
Our programs can help participating clients significantly improve their diabetes in terms of their quality of life, reduce medication requirements and in some cases, even lead to remission of diabetes.
Our Programs
Our weight management programs are led by professional experts including physicians and registered dietitians. We aim to provide tailored solutions for weight management based on your medical condition, cultural preferences, and baseline body weight.
Our programs are based on small, bite sized behavior changes that are guided by health professionals and are easy to fit into your current lifestyle. Some of the strategies we teach in our programs include mindful and intuitive eating, dining out healthy, balanced plate, nutrition label reading and meal planning for success just to name a few.

PROGRAM 1: HEAL-kickstartTM
A 12-week nutrition counselling kickstart program offers unique features such as one-on-one appointments with our registered dietitian, culturally tailored structured meal plans, unlimited email support and monthly support groups, and ongoing accountability and goal setting check-ins to help you become successful in your health and fitness goals.
What you get?
– A 12-week nutrition counselling kickstart program
– Six 1:1 biweekly appointment with our registered dietitian, unlimited email support and monthly support groups, and ongoing accountability and goal setting check-ins to help you become successful in your health and fitness goals. The dietitian will provide you with personalized meal plans (including a grocery list and recipes) to help you achieve your weight loss goals. Our meal plans are customized based on your unique health and fitness goals, nutritional needs, and cultural preferences.
Enrollment
This program is open to all our patients and the general public. You do not need a referral, nor do you need to meet with one of our physicians before enrolling.
PROGRAM 2: Heal On
A year long one-on-one healthy lifestyle-based weight management program led by our registered dietitian. This program offers unique features such as face-to-face or virtual visits with the dietitian, personalized meal plans, goal setting and problem solving, monthly group meetings, and nutrition toolkit to help you achieve your weight loss goals.
What you get?
– One-on-one weight management program with registered dietitian (RD)
– Bi-weekly and monthly visits with the dietitian (in person or virtual)
– Personalized meal plans tailored to your cultural preferences and eating habits
– Unlimited email/online support
– Social support through monthly group meetings
– Education on mindful eating strategies to achieve and maintain optimal health and your best weight
– Nutrition toolkit to help you develop healthy eating skills and achieve weight loss supportive behavior modifications
– A coach available to help you overcome roadblocks and challenges

This program is open to all our patients and the general public. You do not need a referral, nor do you need to meet with one of our physicians before enrolling.
PROGRAM 3: Dream 1 – Full Meal Replacement
An intensive, yearlong medically supervised meal replacement and healthy lifestyle based weight management program combines the expertise of Physicians & Dietitians with a high-quality, calorie-controlled, nutritional formula (Optifast® 900) to help you succeed in your weight management efforts.
Program Features
– Medically supervised calorie-controlled program
– Weekly, biweekly, and monthly visits with the dietitian to support you in achieving and maintaining your weight loss
– Coaching sessions available through telephone and video calls
– Personalized meal plans tailored to your cultural preferences and eating habits
– Unlimited email/online support
– Social support through monthly group meetings
– A health professional available to help you overcome roadblocks and challenges throughout the duration of the program
Program Structure
– 24 weekly lifestyle classes
– One-on-one visits with our physicians
– Full meal replacement protocol (Optifast® 900) for 12 weeks followed by a partial meal replacement protocol (Optifast® 900) for 6 weeks as you transition back to self-prepared meals
– Calorie controlled and culturally tailored meal plans to help you transition to self-prepared meals

This program requires referral by a physician. You can access our referral form here. Space is limited and patients are enrolled on a first-come-first-served basis.
One we receive a referral for you from your physician, we will call you to schedule a consult with our registered dietitian, who will review the DREAM program with you and assess your eligibility for the program.
PROGRAM 4: Dream 2 – Partial Meal Replacement
An intensive, yearlong medically supervised PARTIAL meal replacement and healthy lifestyle-based weight management program combines the expertise of Physicians & Dietitians with a high-quality, calorie-controlled, nutritional formula (Optifast® 900) to help you succeed in your weight management efforts.
